In collaboration with the BlueCross Blue Shield of North Carolina Foundation, Prevention Institute profiled healthcare facilities that are implementing CCHH principles. It has also examined the actions of Community-Centered Health Homes in addressing community conditions to improve health and reduce illness and injury.
